Not all nursing homes, assistant living and independent livings are the same, let us make the process easier for you. Our transition experts will help you find the right setting and facility for you, inspections will be done by qualified therapists of prospectives placements.

Aging in place services : our experience therapists will come out to your primary residence and assess it to ensure you have what you need to continue to live at home/in the community safely and prevent falls. We will also provide recommendations for adaptation/remodeling to make your home safe for you. Our goal is that you will stay in your own home for as long as you want to safely. Going back home after hospitalization? we can help with that process as well.

Payment Information :
We offer many payment options depending on the services rendered .
We do not accept insurance because of the restrictions insurance places our services and our patients under. This also allows us to fully focus on you, the client and your needs instead of insurance companies rules. There are many useful services that INSURANCE DO NOT PAY FOR INCLUDING OURS.
We aim for a worry free process. Due to this reason, we only take cash, credit card and/ fsa/hsa accounts. Please check your out of network benefits as well.
For virtual services : $70 an hr ( education service)
For Home safety assessments and recommendations - $100 an hr ( it will take an hr to complete)
For transition, navigation and community re integration services - $85 an hr
We do ask for a 1/2 deposit prior to services rendered and full payment once completed.
